Completing an Assessment

This page covers everything that happens inside a review or checklist once you have started it — navigating categories, answering every question type, attaching photos, capturing signatures, flagging corrective actions, and submitting.

Assessment Overview Screen

After opening a review or checklist, you land on the Assessment Overview. This screen shows:

  • Completion percentage — how many questions have been answered out of the total
  • Template name — the evaluation template you are using
  • Location and region
  • Start date
  • A list of all categories in the template

Each category row shows:

  • Category name
  • How many questions are answered vs. total (e.g. "3 of 8")
  • A warning icon if any required questions are unanswered
  • A checkmark when the category is fully complete

Tap a category to open it and begin answering its questions.

Answering Questions

Inside a category, questions are displayed one at a time. Use the Previous and Next buttons at the bottom to move between questions, or tap a breadcrumb to jump to a specific one. A progress bar at the top shows your position within the category.

Each question shows the question text, any descriptive text, and the response area. Questions marked with a required indicator must be answered before you can submit.

Tap the ? or info icon on any question to open a helper panel with additional context or instructions.

Question Types

Yes / No / N/A

Tap the button that matches your answer. The selected button highlights.

Pass / Fail

Tap Pass or Fail. A Fail answer may automatically create a corrective action item — see Corrective Actions below.

Single-Line Text

Tap the text field and type your response. Use this for short answers like a name, ID number, or brief observation.

Paragraph Text

Tap the text area to open a larger input field for multi-sentence notes or descriptions.

Select One

A list of options appears as radio buttons. Tap one option to select it. Only one answer is allowed.

Select Multiple

A list of options appears as checkboxes. Tap as many as apply.

Date

Tap the date field to open a calendar picker. Scroll to the correct month and tap the date.

Date and Time

Tap the field to open a combined date and time picker. Select the date, then adjust the time using the hour and minute scroll wheels.

Time Only

Tap the field to open a time picker. Scroll to the correct hour and minute.

Numeric

Tap the field and type a whole number using the numeric keyboard.

Decimal

Tap the field and type a number with decimal places (e.g. 98.6).

Money / Currency

Tap the field and enter a dollar amount. Do not include the currency symbol — just the numbers.

Temperature

Tap the field and enter a temperature reading. The app applies the range thresholds configured in your template and flags out-of-range values automatically.

Warning

Temperature readings that fall outside the acceptable range will be flagged as a violation and may generate an automatic corrective action.

Signature

A blank signature pad appears. Use your finger or a stylus to sign. Tap Clear to erase and start over. Tap Done or move to the next question when you are satisfied with the signature.

Asset

A searchable dropdown lists the assets configured for your organization (equipment, fixtures, etc.). Tap the field, type to search, and select the correct asset.

Photo Only

This question type requires at least one photo before it can be marked complete. See Taking and Attaching Photos below.


Taking and Attaching Photos

Photos can be attached to any question — some questions require them.

How to Attach a Photo

  1. On the question, tap the camera icon or Add Photo button.
  2. Choose one of:
    • Take Photo — opens your camera to capture a new image
    • Choose from Library — opens your photo library to select an existing image
  3. After selecting or capturing, the photo thumbnail appears below the question.

You can attach multiple photos to a single question. Tap a thumbnail to view it full-size or remove it.

Saving Photos to Your Camera Roll

If you want copies of review photos saved to your device's photo library, go to the Menu → toggle "Save Photos to Camera Roll". This applies globally to all photos taken within the app.


Corrective Actions

A corrective action is an action item linked directly to a question. They are created in two ways:

  1. Automatically — when a question with a configured failure threshold receives a failing answer (e.g. a Fail response or an out-of-range temperature)
  2. Manually — you can add a corrective action to any question at any time

Viewing and Editing a Corrective Action

When a corrective action is attached to a question, a flag icon appears on that question. Tap the flag to open the action item details:

  • Description — pre-filled with the question context; you can edit this
  • Assigned to — select a team member responsible for resolving the issue
  • Due date — set a deadline for resolution
  • Photos — attach photos documenting the issue

Tap Save to confirm. The action item is tracked in the Action Center and on location reports until it is resolved and approved.


Conversation Notes

On any question, you can add a text note for context that does not fit neatly into the answer itself. Tap the note icon or Add Note on the question, type your observation, and save. Notes are visible to anyone viewing the completed assessment later.


Navigating Between Categories

You do not need to complete categories in order. Tap the back arrow to return to the Assessment Overview at any time and choose a different category. Your answers are saved automatically as you go.


The Actions Menu

The three-dot or dropdown Actions menu at the top of the Assessment Overview gives access to:

Submitting an Assessment

When all required questions are answered, the Submit button becomes active.

  1. Tap Submit.
  2. A confirmation dialog appears summarizing the assessment. Review it.
  3. Tap Confirm Submit.

The assessment is uploaded. Its status changes to Submitted (or Pending Approval if your template requires manager sign-off), and it moves from In Progress to the Past 7 Days segment on the dashboard.

Tips

You cannot edit a submitted assessment unless a manager or admin unlocks it for revision from the web portal.

Incomplete Submissions

If required questions are still unanswered when you tap Submit, the app highlights the incomplete questions and prevents submission. Return to each flagged category and complete the outstanding answers.


Auto-NA (Automatic Not Applicable)

Some templates are configured with Auto-NA logic. When a question is marked Not Applicable, related sub-questions may be automatically set to N/A as well, saving you from answering questions that are no longer relevant. This happens automatically — you do not need to do anything extra.


Repeating Questions

Some questions are configured as repeaters, meaning you can log multiple instances of the same data point. For example, a temperature log question might allow you to record readings for multiple coolers. Tap Add Instance below the question to add another row, fill it in, and repeat as needed.
