Reviews
Reviews are the core feature of ActionCard. A reviewer visits a location and works through a structured evaluation template, answering questions, attaching photos, and flagging issues as action items.
Starting a New Review
- Navigate to Reviews in the left sidebar.
- Click New Review (or tap + in the mobile app).
- Select the Template you want to use.
- Select the Location being evaluated.
- Select the Date/Time the review applies to (defaults to now).
- Click Start Review.
The review form opens immediately.
The Review Form Layout
The review form has three panels accessible via tabs:

Answering Questions
Each question may support different response types depending on how your template is configured:
- Yes / No / N/A — click the appropriate answer
- Pass / Fail — used for compliance-style questions
- Score — numeric entry
- Text — free-form comments
- Photo required — you must attach at least one photo before the question is considered answered
Questions with a red indicator are required. The Completion percentage at the top updates in real time as you answer questions.
Attaching Photos
Photos provide visual evidence for a response and are often required for certain questions.
- On the question you want to document, click the camera icon or Add Photo.
- On mobile: choose to take a new photo or select from your library.
- On desktop: upload an image file from your computer.
Photos are stored with the review and can be viewed later in the Image Gallery.
Flagging Action Items
If a question reveals something that needs corrective action, you can attach an action item directly to that question.
- While on the question, click Add Action Item (the flag icon).
- Enter a description of what needs to be corrected.
- Assign it to a user responsible for resolution.
- Set a due date.
- Click Save.
The action item is tracked in the Action Center and on location reports until it is resolved and approved.
Leaving Conversation Notes
On any question you can add a private note for context or follow-up discussion. Click Add Note, type your comment, and save. Notes are visible to other reviewers and managers viewing the completed review.
Completing and Submitting a Review
When all required questions are answered:
- Click Finalize or Submit Review.
- If your role requires approval, the review will enter a Pending Approval state until a manager approves it.
- Once finalized, the review appears in reports and the activity feed as completed.
Tips
Incomplete reviews remain in the In Progress state. You can return to them at any time from the Reviews list.
Viewing Completed Reviews
Go to Reviews in the sidebar. Use the time filter to switch between:
- Past 7 days — recently completed reviews
- In Progress — reviews that have been started but not finalized
- Next 7 days — scheduled upcoming reviews
Click any review to open the read-only detail view showing all answers, photos, scores, and attached action items.
Scheduled / Recurring Reviews
If your organization uses recurring reviews, they will appear automatically in the In Progress view when the scheduled window opens. Recurring reviews are managed under Reviews → Recurring.
